The recovery model at the Colorado Coalition for the Homeless is evidence-based and unlike any other program in the state. Our unique model, used at both the Fort Lyon Supportive Residential Community and the Sage Ridge Supportive Residential Community combines transitional housing with peer support, educational, vocational, and employment services for Coloradans experiencing homelessness with a substance use disorder. Individuals arrive on campus dedicated to recovery, and are encouraged to stay for up to two years to resolve substance use issues with drugs or alcohol, and ultimately attain health stability and housing. 

In addition to our residential treatment programs, the Medication Assisted Treatment (MAT) Clinic at the Stout St. Health Center is designed to be as low barrier as possible with immediate walk-in options, group and peer support, and medical treatment in partnership with ongoing recovery support.
